There's loads of ways to overcome the fact Sheogorath doesn't have a single identity anymore. Look at what they did with Oblivion and the Nerevar, only talking about him but not showing any thing to visually show what he/she looked like.they never even identify the six if I am not mistaken. Same thing could happen for Skyrim.Now if they want to actually show something, a statue or something similar, it seems the easy fix would be to make the statue but have it be in poor condition or destroyed to the point it's not recognizable what type of character is shown.Yet another idea I have heard is that many people in the mortal world may not even know he was replaced.

Just about anything is possible in light of the fact that the premise is based on something that makes no logical sense in our actual physical reality, but is based on Daedric magic, and in the realm of insanity no less.I think one good way to integrate the events in TES4 with how Sheogorath appears now would be to leave his statue looking like it used to look. The statue is, after all, not the god himself, but a hunk of rock that worshipers carved that was probably there since long before the events in TES4. Few return to Tamriel once they would have travelled far enough into the Shivering Isles to encounter the new madgod to be able to travel back and update all the statues with the new look. This eliminates the worry of how the statue should look based on what character actually was the HoK.As for the voice, maybe the shrine doesn't do anything. Maybe because the new madgod started life as a mortal, he can't be summoned at a whim like other Daedra, and doesn't answer prayers anymore. Make his worshipers have dialogue that indicates there is a 'maddening' absence of their master's voice lately.
Or, have it sound just like the old Sheogorath, but don't explain it. That doesn't mean that the HoV has gone mad necessarily, but has decided to step into the role for consistency sake. Or maybe he has gone mad.
